Tetralogy Risk Score (invasive)

This scoring system was devised to assess risk of appropriate ICD shock in primary prevention for patients with repaired tetralogy of Fallot. The system includes results of invasive cardiac electrophysiology procedures and measurement of LVEDP: if these measurements are not available, use the non-invasive risk score calculator.

Khairy et al. Implantable Cardioverter-Defibrillators in Tetralogy of Fallot. Circulation. 2008;117:363-370.

If there is uncertainty in an individual patient, other potential risk factors to consider may include: sustained atrial arrhythmia; reduced RVEF and/or LVEF; fragmented QRS; peak VOand VE/VCOslope; raised BNP; extensive late enhancement on cardiac MRI.

Prior palliative shunt?

Inducible sustained ventricular tachycardia?

QRS duration ≥ 180 ms?

Ventriculotomy incision?

Non-sustained ventricular tachycardia?

LVEDP ≥ 12 mm Hg?
