Using the 3 Sentence Health Summary

It’s good to be able to describe your health condition quickly and clearly

Imagine having to go to the Hospital Emergency Department because you got injured playing sport. Imagine you were staying at your nana’s place and you got appendicitis. Believe me, it happens!

At some stage you will need to be confident describing your health without your parents to help you explain. The 3 sentence health summary is a good place to start. Give it a try!

1

First up,  your name, age, diagnosis and brief medical history

2

Second,  any medications you are taking

3

Third,  the main problem that you are concerned about at this visit

“Kia Ora, my name is Anahera”

”I’m 18 and I’ve had a Fontan operation for a heart condition called tricuspid atresia

I’m taking warfarin

I’ve come to hospital because I’ve got a really sore stomach and I’m worried I could have appendicitis“

“Hi, my name is John”

“I’m 16 and I’ve got Tetralogy of Fallot. I had a heart operation when I was 6 months old

I’m not taking any medicines at the moment

I’ve come to hospital because I was playing soccer and I injured my ankle in a tackle”
